When you walk with a bag slung over one shoulder, you can feel yourself leaning that way. Before I looked into this, I assumed that feeling led straight to "this is how spines get bent out of shape."
The first half of that turned out to be measured. The second half is far more cautious than I expected.
① The tilt is not in your head
What happens the moment you load one side has been measured more than once. In nineteen people carrying 15% of their body weight on one shoulder, the muscle activity on the left and right sides of the back clearly diverged. In another experiment, the center of pressure shifted toward the loaded side.
The same held outside the lab. Twenty-six adults walked for ten minutes in ordinary settings with a backpack, a shoulder bag, and a crossbody bag, and their walking speed and left-right balance changed with each.
So the sense that you are tilting is an accurate observation. The trouble starts after that.
② A bent spine is a separate question
Scoliosis gets attached to bag talk constantly. But the condition's own name answers the question: the adolescent form is classed as idiopathic, meaning no specific cause can be identified.
The international guidelines for its conservative care put it plainly — by definition, idiopathic scoliosis is of unknown origin and is probably due to several causes.
That is not the same as saying the bag theory has been disproven. It means the cause itself has not been established, and no evidence was found that points to bags.
③ On weight, the research is large and found nothing
A 2018 systematic review pulled together 69 studies covering 72,627 participants. Giving the most weight to the five that followed people over time, it found no evidence that bag weight creates new back pain.
In 2020 a different team went further and obtained individual participant data to recalculate. Children carrying more than 10% of their body weight showed no difference in back pain from those who did not. Since every study in that pool was a snapshot taken at a single point in time, the authors themselves rated the certainty as low.
What stands out is less the estimate than the spread. Eleven studies from Saudi Arabia, Iran, Brazil, Spain, Portugal, New Zealand and elsewhere, and the heterogeneity between them was zero. There was almost no room for the result to have landed there by chance.
And that "10% of body weight" figure was never measured into existence. The 2020 paper says so directly: it is a threshold that clinicians and a handful of professional bodies converged on. Guidelines put the safe load anywhere from 5% to 20%. Drawing the line there did not divide anyone — that is the more precise way to say it.
④ The one-shoulder question itself has not really been tested
The conclusion of that 2018 review lists carriage method alongside weight. Open the paper, though, and the five studies that followed people over time looked at weight and at how heavy the bag felt. None of them looked at how it was carried.
Carriage method came up only in the snapshot studies, and three of them did report a link between carrying on one side or asymmetrically and back pain. The other twenty-nine found none.
A study published this year, following 2,423 Chinese adolescents, points more sharply: students who carried on one shoulder had roughly double the risk of back pain. But the exposure and the pain were both self-reported, and the authors state outright that they cannot rule out students who already had pain having switched to one shoulder because of it, so causation cannot be established.
So: on weight, the answer is close to "no association." On carriage method, it is closer to "never properly tested." Those are two different kinds of not knowing.
⑤ One thing did track, consistently
This is the part of the 2018 review I stared at longest. Actual weight showed no link to back pain — but whether the bag felt heavy did.
Children who said they struggled to carry their bag were about twice as likely to have back pain persist, and those who described it as heavy were similar. The authors graded this evidence as low, and there is room for pain to be what made the bag feel heavy, but nothing here argues for flipping the direction.
Which is to say: "does this feel heavy today" is a more useful signal than the number on a scale.
⑥ So what actually helps
▸ If you carry on one side and nothing hurts — there is no evidence it is bending your spine. The tilt is real and measured, though, so switching shoulders whenever you stop at a crosswalk is worth doing. It costs you essentially nothing.
▸ If your shoulder or neck aches afterward — before cutting weight, look at how long you held one position. What the experiments above measured was mostly walks of around ten minutes.
▸ If it often feels heavy — that feeling was the signal that tracked most consistently in the data. That is the day to take something out. The same goes for a child's bag: listening beats weighing it.
Changing the question helps. Not "which shoulder should I use," but "how long have I been like this."

The evidence on pain comes largely from children and adolescents rather than adult commuters. The experiments measuring the body's immediate response do include adults, but in small samples. The two reviews searched the literature up to 2016 and 2019 respectively.
